When instructed that a member of theanaestheticgroup will be acting as your anaesthetist, either by the hospital where you are having your procedure or the surgeon performing your procedure, we will need to collect personal and medical information for planning your care, billing and communication purposes.

This information will be kept (password protected) and communicated securely (encrypted), and will not be distributed, sold or divulged to anyone other than those parties involved in your medical care. This may include the hospital, surgeon, medical secretaries and medical insurance companies if you have a policy with one. Any personal information shared will be kept to the minimum possible and will only be shared to facilitate your treatment. Any data collected may be kept for up to 7 years for tax purposes. Any data collected will be deleted or destroyed securely as soon as it is no longer required.

Consent

By instructing or having a member of theanaestheticgroup act as your anaesthetist for your medical procedure, it dictates that you are consenting to your personal information being collected and used as detailed above.

Review

The policy was written in May 2018 and will be reviewed every two years, or earlier if there is a statutory requirement to do so.
